IMPORTANT NOTICE
We are no longer accepting Medicaid (Apple Health), Medicare, or TriCare.
We can not accept Duo Complete plans that include Medicaid.
If Medicare is your secondary insurance, we may still be able to see you. However, you will be responsible for any remaining patient balance after your primary insurance processes
the claim.
INSURANCE INFORMATION
We accept most Blue Cross Blue Shield plans, including Regence, Premera, BlueCard and Anthem. We are Preferred providers in The First Choice Health Network . We also accept some Aetna and Cigna plans. However, we are only able to accept certain Open Access plans. We are not Aetna Signature Preferred Provider Network. So we will need to check your specific plan first.
Coverage and benefits can vary significantly, and in some cases patients may be responsible for a larger portion of the cost.
We’re happy to review your specific plan with you and help you understand what to expect before your visit.
NEW PATIENTS.
when filling out an appointment requests, PLEASE BE SURE TO INCLUDE:
Insurance plan name,
Insurance Plan ID including letters.
Select a reason for visit. When reason for visit is blank, the request declines.
Unless you have a medical need to do so, we do not perform Gyn Procedures or Annual exams on New Patient visits.
For Returning Patients
Be sure to uncheck the "New Patient" box when scheduling. This should bypass the insurance information step. We appreciate your understanding and look forward to partnering with you in your healthcare journey!
